EE presented to EH clinic with unsteady gait and stated on [REDACTED] at [REDACTED] she was in the EC cleaning room 4a. EE stated pain 6 10. EE denies ibuprofen. EE stated she took [REDACTED] for pain. EE provided heating pad. EE stated she was mopping the floor and pushed the bed to mop underneath. EE stated when she was done mopping the area she then tried pulling the bed back and then she felt a pain in her lower back. EE stated she informed her leader [REDACTED] [REDACTED] and her leader dismissed her injury. EE informed of WRI process and the importance of completing an event report with 24 hours of WRI. EE c o radiating pain from right lower back down right leg.
