What happened
Employee was on the 8th step of a 10 ft ladder with his bucket picking cherries. He already filled up his bucket but he was missing just a few more at the top of the tree. He stretched up to grab the cherries and the weight overcame him. The top of the ladder twisted and broke causing the employee to fall forward and landed on his stomach with the bucket.
