On [REDACTED] at [REDACTED] EE was using a Sioux [REDACTED] 4 1 2 angle wheel grinder to clean up the edges of tube 131777 for placement of cap 131782 when EE grinder wheel broke off causing it to kick back resulting in 3-4 laceration approximately 1 4 deep on exterior side of left hand between fingers.
