Patient was attempting to stand then transfer with assistance from [REDACTED]. Upon attempting to stand patient stated his knee gave out. Patient sat on slight edge of bed then attempted to scoot his bottom back onto bed instead began slipping off. [REDACTED] attempting to stop patient from sliding helped him as she could to the floor. [REDACTED] was immediately in alot of pain from injury to back.
