Before the incident
Employee was attempting to unjam tape from an automatic tape dispenser.
What happened
Employee turned the taper off (switch on back) then flipped the unit on it 's end so she could see into the discharge. In doing so she inadvertently turned the taper back on and when she stuck her finger into the discharge to remove the jam the automatic tape blade actuated and cut her finger.
