Before the incident
Employee was taking a hilo certification test here by our internal hilo trainer. He was driving into trailer through our receiving dock door to collect a few empty pallets then exit the trailer with them.
What happened
as the driver exited the trailer he panicked and turned sharply after exiting the trailer. there are racking systems over the dock doors so the panicked sharp turn lead him straight into an upright of the rack
