WHEN I WENT TO EXIT THE BED OF THE PICKUP TRUCK [REDACTED] I WAS FACING REARWARD TOOK A STEP DOWN ONTO THE BUMPER STEP PAD AND AS SOON AS I PUT MY WEIGHT ON THE BUMPER STEP PAD AND WENT TO BRING THE OTHER FOOT FORWARD TO STEP DOWN THE STEP PAD (BLACK PLASTIC COVER) DISLODGED FROM THE BUMPERS STRUCTURE AND SLID OUT FROM UNDER ME AND SENT ME FLYING FACE FIRST TO THE GROUND. I PUT BOTH HANDS AND ARMS OUT FORWARD TO BREAK MY FALL AND AVOID HITTING MY FACE ON THE GROUND. I HIT WITH SUCH FORCE ON THE LEFT SIDE THAT MY HAND ARM GAVE WAY SENDING ME SHOULDER FIRST (LEFT SHOULDER) INTO THE GROUND AND ENDED UP ROLLING END OVER END TO MY LEFT ONCE CAUSING ABRASIONS TO MY LEFT SHIN APPROXIMATELY 6 INCHES BELOW THE LEFT [REDACTED] APPROXIMATELY 2 INCHES BELOW MY LEFT ELBOW ON MY FOREARM AND LEFT PINKY KNUCKLE . WITHIN MINUTES I FELT MY LEFT SHOULDER AND NECK AREA STARTING TO GET SIGNIFICANTLY STIFF. THIS IS WHAT CONCERNED ME AND PROMPTED ME TO CALL THE COMPANY NURSE AND FILE A REPORT. WITHIN AN [RED
