Assisting Patient to the restroom with RN
What happened
pt stated that she needed to use the bathroom. I unplugged SCD 's from pt legs placed non-slip fall risk socks on pt while pt was still laying in the bed. Assisted [REDACTED] RN and pt to side of the bed in a sitting position and held walker in place for the pt to hold while RN was assisting pt into a standing position. pt stood at bedside and stated that they felt stable and also appeared stable. pt started to push the front wheel walker with RN right next to her side helping her to use the walker with one had as pt was unable to use left hand due to hand in sling. pt and RN reached the bathroom doorway and I held onto the IV pole following pt and RN. pt then fell with the walker onto the bathroom floor. As pt fell IV pole was pulled toward pt and IV line then became tangled in pt legs. When the IV pole was pulled forward my left arm was in the doorway pulled forward putting pressure on my Left shoulder.
