Before the incident
Taking a piece of equipment to the scrap bin. Using fork truck to move the piece of equipment to the scrap bin.
What happened
While moving a piece of equipment the piece of equipment tipped to the side. Emp shut down fork truck and exited. Emp noticed the fork was tilted up. He did not realize that meant it was unhooked from the bottom track of the mast and only supported by the top of the mast. Emp went to adjust the forks the left fork fell and struck his foot.
