What happened
The employee removed the die from [REDACTED] and was in the process of tearing it down. After taking the pins out of the stripper it should have slid from the top of the die onto the die cart. The employee grabbed the stripper from one end. There wasn 't enough clearance and his finger got caught between the die shoe and the stripper.
