What happened
Prior to employee arriving on the scene the operator and supervisor were reviewing the procedure of changing filters. The valves were closed. The filter press remained full of plating solution. Employee arrived discussed emptying the press. Employee opened a couple of valves. The pump was activated causing the plating solution spray out from the loose seal covering the operator.
