Before the incident
[REDACTED] was locating a pallet into a bin when the pallet he made contact with a pallet in the bin he was attempting to place his pallet into. One of the straps on his pallet broke pushing the cartons on the pallet to fall to the floor. When the cartons hit the floor at least two cartons made contact with [REDACTED] 's chest arms and abdomen. The force of the cartons striking him caused him to fall backward inside the reach truck operator compartment injuring his back as well.
