What happened
[REDACTED] aggressive and yelling when the nurse went to introduce herself. Situation kept escalating. Security was called. [REDACTED] was screaming in the doorway of her room and refused to go back into her room. Staff continued to try to calm pt down and talk patient down. [REDACTED] continued to become increasingly angry. We told patient we will have to give her a medication to help calm her down if she did not go back into the room. The patient continued to cuss at staff and call staff names. One nurse pulled a dose of [REDACTED] [REDACTED] we asked the patient again to go back into the room and stop yelling. [REDACTED] non compliant. Nursing staff and security all stepped into the room to give the patient the medication we had to hold pt down to give the med the pt became even more upset. When everyone got up off the floor the patient quickly punched [REDACTED] the nurse in the neck and [REDACTED] got slammed against the wall.
