At approximately [REDACTED] on [REDACTED] a contract employee working in the sugar warehouse sustained a laceration to his finger while using a slide hammer to remove a shaft from the upper feed table support wheel in [REDACTED]. The employee requested medical attention and was treated by the on-site nurse who bandaged the injury. Cause of Injury: The injury occurred when the employee’s finger became caught between the brass sliding component of the slide hammer and a nut welded to the end of the ½” rod. The employee was wearing gloves at the time of the incident. The slide hammer in use was a shop-fabricated tool consisting of a ½” steel rod with welded nuts on each end and a 2” diameter brass hammer that slides along the rod. Notably the brass hammer lacked protective edges or guards to prevent hand slippage contributing to the injury.
