Before the incident

picking shingles outside in the shingle yard and while lifting a bundle he felt a sharp pain in his upper back. [REDACTED] then felt a lot of pain when he breathed and moved. He then came and reported the injury to myself and Ops Manager [REDACTED]. We printed out the appropriate paperwork and took him to the Urgent care.
