EE was traveling on a fork truck to warehouse to replenish trash bags. EE cut across the front hallway in [REDACTED] in front of hot packs.
What happened
An I-Beam was in the EE 's Blind spot of fork truck mast. EE hit the I-beam at full speed with fork truck. EE was jolted into the mast with their shoulder and head into top frame above seat. Seat broke off track from fork truck.
