What happened
Employee was on the ladder approximately 3 ' of the ground cutting roof membrane from a side wall. When he tried pull the cut piece down it was still slightly attached so he pulled a little harder. When he pulled harder the material let loose and he lost his balance causing him and the ladder to fall sideways. When he fell his left foot slide between the steps of the ladder.
