What happened
Two teammates were working the winder. They went to put the tape in on the back of the line for tape sample in. [REDACTED] was at the winder with another teammate. The winder was indexed already but the tape but put on incorrectly. The winder was already indexed at that point. The teammates reached over the already indexed core to get the cover off of the two-sided tape. The tape tore halfway down the core. [REDACTED] leaned her hip on the core where the chuck enters the core. Her pants got caught in between the chuck and the core as it was turning her clothing got caught and the winder pulled her into the spindle.
