What happened
The employee grabbed the grinder to cut the tube. As the employee leaned in to view where to make the cut he activated the 2nd trigger with his body. This is a 2-hand control grinder. As he did both triggers were activated and the cutoff wheel came in to contact with his hand through the glove.
