Before the incident
EE was doing a product filling trial on line 6 while he was waiting for the jars to move on the capper outfeed conveyor one of the bottles fell and he tried to pick it up while the conveyor was moving.

Unsafe act the EE tried to pick up a fallen bottle while the conveyor was moving. He did not stop the machine and he did not use safety gloves.
