What happened
A Bracket Prep associate was adjusting the brackets on a cart in their area when the operator plate that they were adjusting at one end of the cart slipped through the gap at the front of the cart and landed on their Right Foot. The bracket involved was found to be approximately 33 lbs. Associate was wearing their safety toed met guard shoes but they were an older pair the associate had been wearing regularly for over a year. The height from which the operator plate fell was approximately 37’ high.
