Changing a dry pump filter on the [REDACTED] furnace

On [REDACTED] an employee at the Alloy Service Center (ASC) sustained second-degree facial burns and eye irritation due to a magnesium flash explosion while changing a dry pump filter on the [REDACTED] furnace. The incident occurred despite the employee wearing all required PPE. The explosion was triggered by magnesium buildup in the filter likely ignited by electrostatic discharge during an o-ring replacement. Contributing factors included overheating of the melt caused by heavy dross accumulation which led to a reduction reaction converting magnesia to magnesium. Immediate corrective actions included halting melting of large slugs from furnace failures vendor cleaning of vacuum systems and initiating evaluations of filtration systems PPE updates and process risk assessments.
