Entered Rm 71 to give her night medications went in rm placed meds on dresser to turn light on. Observed she was in aa sinking position Writer moved bed up to evaluate issue with mattress.

EE on a mat next to bed turned right to check to see if connection popped off motor. next thing EE knew hit the mat chin hit mat forcefully then my head hit the floor.
