He stated that as he was changing over a jig that is located above his head he needed to locate a steel plate (approximately 16 lbs) into 2 or more sliding clamps which sets the spacing between them.

As he was adjusting these clamps he stopped supporting the plate with either hand assuming that its locating slots (keyhole style) would hold it in place. The plate either popped up from the slot(s) or wasn 't fully seated and became uncontrolled striking him in the nose forehead.
