Before the incident
The operator disconnected the high-pressure hose and proceeded to install a 90-degree fitting to blow dry and remove the remaining test water from the tubing.
What happened
After rotating the hammer nut onto the 90-degree fitting and believing the fitting was partially threaded he began to release his grip the fitting disconnected and fell approximately five inches pinching the operator’s right ring finger between the fitting and the inside of the spool
