She raised the guard to engage the interlock; a second TM who is normally not there was standing to [REDACTED] 's right (where she usually stands); [REDACTED] rested her left hand on the lip of the machine carriage track; The second TM manually moved the carriage without checking and the carriage crushed [REDACTED] 's pinky between the carriage and inside lip of the machine. - documented from interview with [REDACTED] at the machine on [REDACTED]
