Before the incident
TM was using a 20 Ton crane to flip over a crusher dust bonnet (~300 lbs.) that was resting on the shop floor after replacing the seal.
What happened
As dust bonnet become vertical and started to pivot toward TM (as intended) one of the hooks detached from the lifting eye (unintended) causing the dust bonnet to fall onto the TM 's left foot.
