Before the incident
Employee entered builidng from outdoors during snowy weather employee did not change out of wet outerwear. Carrying her personal items (i.e. purse lunch bag paperwork) employee walked down the tiled hallway to unlock her office door.
What happened
When employee reached forward to insert the key in her office door her personal items pulled her off balance and paired with the employees wet outwear shoes the employee fell forward onto the tiled hall floor striking her elbow on the floor.
