Another employee went over a weak spot of the deck and punctured a 1 ¼” hole through the deck causing the tank below (6-220-1-F) to leak out fuel at a PSI range of approximately 8. Fuel began to cover the deck creating a slippery surface employee slipped and fell onto the deck.
