Caregiver reported to me (her supervisor) that she hit the back of her head quite hard on the breakroom refrigerator and was now experiencing a headache. Caregiver instructed to sit a bit and if feeling worse to check in (Urgent Care) for evaluation. [REDACTED] caregiver approached me stating she was now dizzy. Caregiver clocked out and checked in for evaluation and was diagnosed with contusion to her head and cervical muscle strain.
