Before the incident
EE was taking a smock cart to the Smock room.
What happened
The EE was taking a cart of smocks to the smock room. There was a forklift nearby the operator stopped and the wheels on the cart got stuck onto the forks. As the EE was attempting to remove the cart the operator reversed the lift and the front wheel scraped the EE 's left ankle.

Immediate swelling and bruising of left foot and left pinky toe as well as scrape injury of left foot due to being struck by wheel on forklift.
