Before the incident
EE got called over to bag machine [REDACTED] for trim issues. EE was troubleshooting why there was no suction on the trim vacuum. EE walked around to the backside of the machine and pulled out his flashlight and bent over at their waste to see if the blades on the trim chopper were spinning.

When EE bent over he struck his head on the shut off valve for the vacuum suction.
