Employee was walking north in the walk area between Side A and Side B from the double doors between the Bakery room and the Fill and Seal room She was carrying an empty white tub in front of her bringing it back from the Bakery stick sorter to put it in the Side B lateral expeller reject location As she got closer to the eye wash station she was unaware that there was water on the floor in the walk area A Maintenance Tech had accidently triggered the paddle on the eye wash station while working on equipmentand some water spilled from the eye wash station onto the floor in the walk area At the time the Maint Tech did not realize that he had touched the paddle or that there was water on the floor The employee carrying the white tub did not see the water slipped when her foot contacted the water and she fell backwards onto her behind on the floor As the employee fell her left jaw neck and left shoulder contacted the conveyor and electrical box in front of the metal detector on [REDACTED]
