Before the incident
Employee was packing tortilla packets onto a wooden pallet. When the pallet was full of product she walked towards a pile of empty pallets to get a replacement pallet.
What happened
Employee took a step backward while holding an empty wooden pallet and stepped on a manual pallet jack that was in motion in immediate proximity behind her as it was being pulled by another employee. Employee lost her footing fell to the ground and injured her knee and ankle.
