On [REDACTED] at [REDACTED] she had three-point contact holding on the railing with her right hand carrying items in the left arm. She was wearing tennis shoes with new inserts. Her left foot twisted backwards with her toes staying on the step and this caused her to lose her balance falling forward. Her supervisor is [REDACTED] She has done this mobile job for three years.
