Before the incident
Employee was dropping off GFM material to be used by a subcontractor to be installed on the ship. SKTR did not bring forklift to the front gate for off flatbed side pickup & did not respond to phone call. Alternative method of delivery used. Employee backed the flatbed to the receiving area and needed to rotate the pallets of the material 90 Deg so it could be offloaded from the truck via forklift on the back of the flatbed.
What happened
Employee was using a material handling hook and pulling the right corner towards themself as they were standing on the left edge of the wet (Rainwater) flatbed. Employee 's hook slipped off the material causing a loss of balance and backwards momentum leading to the employee falling backwards off the flat bed falling roughly feet to the asphalt landing on their buttocks then rolling and slaming their hardhat (Which was on their head) onto the ground.
