[REDACTED] alerted RN chemotherapy never administered. [REDACTED] went to local ER for disconnectflush of mediport. RN opened the sharps container to assess the chemotherapy. Mediport was not properly de-accessed safety retract not completed and needle was slightly exposed but wrapped within the dressing unable to see needle at the time. RN tip of pinky got stuck with mediport needle.
