After my shift on [REDACTED] at [REDACTED] I was entering the parking garage 1 on 4th floor. As I stepped off the elevator I tripped and fell to the ground mainly on my left side. There were 2 Nurses that helped me off the ground. I am unsure of their names. I would like for footage to be looked at involving the fall. After looking at the elevator on the right side the floor is uneven while stepping off.
