What happened

Called to room by patients primary nurse [REDACTED] who was requesting staff assistance with patient who was getting upset due to the fact that he is ready to leave. [REDACTED] had just assisted patient to the bathroom and have given the patient nutrition but patient was upset and started to punch staff and the walls. Staff was trying to redirect patient and help him to not hit his head due to him recently having surgery. Security called to room to help assist with staff due to patient being aggressive. Security unable to calm patient. Patient had to be lowered to ground by staff and then carried by staff to bed. At this time patient was still kicking biting punching and spitting at staff. Patient punched nurse [REDACTED] in the face and ribs during this time. Patient punched nurse [REDACTED] in the leg. Patient also kicked security guard in the face.
