__R[REDACTED]ACT[REDACTED]__ [REDACTED] RN __R[REDACTED]ACT[REDACTED]__ She received a flu shot on __R[REDACTED]ACT[REDACTED]__ at WG Employee Health flu clinic. She states that the personnel who injected her did not put the shot into her left deltoid; it was placed very high up on her shoulder. She stated that it was very painful initially and has continually gotten worse instead of getting better. The pain continued to worsen and she went to see her doctor for her shoulder on __R[REDACTED]ACT[REDACTED]__ and her physician is treating her with medications. The provider she is seeing at Hometown Medicine told her the injection was given into her joint and not her muscle. He is ordering a X-[R[REDACTED]ACT[REDACTED]] to be complete to ensure that the vaccine is still not sitting in the joint space or that a needle did not break off into the space.
