What happened
patient called and stated that she needed to use the restroom and i offered the BC because 1st shift had informed me that she had an episode [REDACTED] that she almost passed out on the toilet so they didnt want her to get out of bed. it took 2 of us to get her to the toilet but once on the toilet she stated that she started getting really hot and was about to pass out. Upon getting her off the toilet it took 3-4 of us because patient provided little to no help with transfer. after we got her in bed 2 of us had to pull her up in bed and thats when I hurt my arm
