On [REDACTED] at approximately [REDACTED] while in the Emergency Department (ED) patient [REDACTED] (DOB: [REDACTED] MRN: 568376158) began to escalate his agitation and bang his head on the door. Mr. [REDACTED] who already had several interactions with security to this point had previously been medicated and due to threats made towards staff had a seclusion order placed and his door was shut. Mr. [REDACTED] refused all commands given by security and medical staff therefor security was forced to intervene. On directions given by medical staff Mr. [REDACTED] was going to be placed in level three four-point restraints which were already placed on his bed. When security entered the room Mr. [REDACTED] began fighting with Security Officers [REDACTED] and [REDACTED]. Officers [REDACTED] and [REDACTED] were securing Mr. [REDACTED] 's upper half and arms with Officer [REDACTED] securing Mr. [REDACTED] 's right leg.
