Before the incident
The temp worker noticed a piece of latex and tried to remove it from under the middle section by reaching into the space between the mold pieces while the upper piece was suspended in the air.
What happened
On [REDACTED] at approximately [REDACTED] a temporary P02C Urethane operator [REDACTED] sustained a partial amputation of the right index fingertip during a demolding process in the urethane area at the [REDACTED] facility. The incident occurred while handling a steel urethane Mold using an overhead crane. The injured worker had been employed for seven days and was undergoing on-the-job training. The assigned trainer briefly left the area to return a forklift access card leaving the trainee unsupervised. During this time the worker took the initiative to operate the overhead crane without authorisation to lift the mould lid. The mould 's middle section remained stuck to the top section. This is a common issue in operation because latex in the mould adheres to the steel to prevent urethane from leaking out.
