Housekeeping cleaning shower area turned and reached for mop an d had sudden onset of pain
What happened
EE arrived at EHS this am stating on [REDACTED] she was at work and turned to pick up mop and had sudden onset low back pain at left hip area shooting down leg. EE states pain was 10 10 and it was difficult to walk. EE states she did notify her supervisor and was out of work yesterday due to pain patient states pain is now 7 10 and appears in a lot of pain is unable to sit well due to pain. EE was given hot pack for comfort EE to see [REDACTED] and will complete RL6 with EHS RN. 0806 EE seen by [REDACTED] to go to [REDACTED] med for evaluation called and gave report to [REDACTED] RN. RL6 completed email sent to supervisor
