Before the incident
[REDACTED] was operating an aerial lift. He was maneuvering the boom of the lift into position to install washers nuts and bolts to secure a horizontal steel girt.
What happened
As he was moving the mobile elevated working platform in reverse he struck the back of his head to a previously installed steel girt underneath his hard hat. His head fell forward on the controls resulting in the boom to extend upward. This caused his jaw and head to get pinned between the girt and the control panel of the lift.
