When he was running to the location he was turning the corner of the facility s laundry room hallway and slipped and fell sliding into the wall outside of the laundry room landing on his right hip area. Community Service was being completed in the hallway prior to the incident. [REDACTED] [REDACTED] reported that the floor was damp at the time of the incident. Wet floor signs were present in the hallway at the time of the incident.
