What happened
EE was cleaning up the SaveAll tank containment area to prevent fiberglass build up which blocks water flow EE used the white water hose to spray residual fiberglass down to the sump pump. While doing this a clump of fiberglass formed preventing the cleaning to be complete EE entered containment area and crawled under the Save All tank discharge tee suction pump piping EE saw they were not going to hit the slide gate valve but did not see the C-clamp and struck the handle pointing down The C-clamp valve handle caused a head laceration through the EEs baseball cap
