Mold changer was at [REDACTED] cavity unbolting the mold that was just set in the press After untying the mold from the spreader bar associate operating the mold truck attempted to back the truck and spreader bar out of the cavity The left edge of the spreader bar caught the guide rod of the press causing the bar to rotate knocking the mold changer off of the ladder onto the floor The mold changer struck his right shoulder and head on the ground then his impact wrench also came out of the cavity sticking him in the face
