[REDACTED] was in the process of taking a scrap green tire from HF6 to designated flatbed for green tire waste across from HF5 using the hand truck on his machine There was a blue bead spacer laying on the hand truck and associate placed green tire waste on top of the spacer When he brought hand truck to waste flatbed associate lifted green tire waste off hand truck and the spacer stuck to tire but fell off on to the floor [REDACTED] stepped on the spacer with his left foot He stated his left foot then slid outwards from his body when he felt pain in his left knee
