Driving a forklift with a pallet of product into a trailer
What happened
Employee drove a forklift with a pallet of product onto a dock plate which was lowered into a semi-trailer. The dock plate fell down a foot from the back of the trailer causing a sudden stop of the forklift. This caused the employee lower back and right knee pain.
